Creativity with Leo Month

At the Brackett Library in Indian River County, Florida, I discovered 11 books to roar into creativity to evaluate healthy goals and achieve health results with Leo month with AI. Library resources may offer ongoing creative learning opportunities. Desha Baker on X: "@HealthyFla I am interested in learning how to engage library resources to serve people of Indian River County, Florida, United States and world to encourage creativity to support, improve community health with Community Health Planning | Florida Department of Health in Indian River" / X. To learn better and evaluate success, I learn with Learn Better. To improve my communication for successful relationships, I learn the importance of speaking clearly with expressive vocabulary with positive body language, welcoming feedback with respect for feelings and views with active listening with I statements of my feelings and views with empathy for needs and commit to organizing solutions. To evaluate goals and micro steps, I learn with One Decision and Your Time to Thrive. To develop healthy goals, I learn with Mayo Clinic Plan, The Fifty Healthiest Habits and Lifestyle Changes with Do This Before Bed. To encourage love with community play to keep sharp beyond anxiety, I learn with How to Love Better, The Way of Play, Keep Sharp and Beyond Anxiety. These books offer learning opportunities to choose a new health goal, evaluate micro steps to the goal and celebrate success with our health communities with AI.

With my challenges of love, I decided to learn with Do This Before Bed. The first practice to learn with is DTBB Resource Chapter 1 to evaluate how energy shifts throughout the day to enhance connections. 7/18 7/19 The Fall Into Wellness program offered with Mayo Clinic recommends to complete 31 daily challenges with 7 to 9 hours sleep, 50 ounces of water, 20 minutes of exercise and write one thankful with a checklist

Three goals for Leo month with my intention of creativity with Leo month to achieve health results are: complete daily challenges with focus on how to do the daily challenges with creativity with Leo month with 7 to 9 hours sleep, 50 ounces of water, 20 minutes of exercise and write one thankful with The Way of Play with our health communities with library media with Morning Check-In, Midday Awareness with Evening Evaluation.

Community Health Campaign to Prevent and Reduce Heart Disease with AI

A Comprehensive Strategy to Tackle Heart Disease

Solving heart disease requires coordinated action across prevention, detection, treatment, research, and technology. Here’s a multi-pronged roadmap to drive down incidence, improve outcomes, and ultimately edge toward eradication.

1. Emphasize Primary Prevention

  • Eliminate tobacco use and exposure to secondhand smoke.
  • Adopt a heart-healthy diet rich in fruits, vegetables, whole grains, lean proteins, and healthy fats.
  • Aim for at least 150 minutes of moderate aerobic activity (brisk walking, cycling) or 75 minutes of vigorous exercise weekly, plus strength training twice a week.
  • Maintain a healthy weight and manage stress through mindfulness, yoga, or other relaxation techniques.

Healthy lifestyle habits—no tobacco, balanced diet, regular exercise—can reduce heart disease risk significantly.

2. Strengthen Screening and Early Detection

  • Begin blood pressure checks in childhood; measure at least once every two years for adults without risk factors.
  • Screen cholesterol every 4–6 years starting in late childhood; increase frequency to annually or biennially after age 40–45.
  • Test fasting blood sugar at age 45 or earlier if overweight or at risk; repeat every three years thereafter.
  • Evaluate sleep quality and screen for sleep apnea to address hypertension and other comorbidities.

Regular screening for blood pressure, cholesterol, and diabetes helps catch risk factors before they progress.

3. Optimize Treatment Pathways

  • Combine heart-healthy lifestyle changes with guideline-driven medications:
    • Statins, PCSK9 inhibitors, or fibrates to manage cholesterol
    • ACE inhibitors, beta blockers, or calcium channel blockers to control blood pressure
    • Antiplatelet therapy (e.g., low-dose aspirin or dual-antiplatelet regimens) when indicated
  • Employ interventional procedures in appropriate patients:
    • Percutaneous coronary intervention (PCI) with stent placement for significant blockages
    • Coronary artery bypass grafting (CABG) for multi-vessel disease
  • Integrate cardiac rehabilitation and patient education to improve adherence and long-term outcomes.

Evidence-based combinations of lifestyle, medication, and procedures dramatically lower heart attack and stroke rates.

4. Accelerate Research and Innovation

  • Invest in novel therapeutics that target underlying biology:
    • Zilebesiran to block angiotensinogen production, preventing hypertension
    • Semaglutide and related agents for weight management and direct cardiovascular benefits
  • Support large-scale trials on new drug classes, gene therapies, and regenerative approaches.
  • Focus on social determinants of health to reduce disparities and ensure equitable access to breakthroughs.

Cutting-edge studies in 2023 showcased novel drugs that outsmart high blood pressure and extend benefits beyond diabetic patients.

5. Leverage AI and Digital Health

  • Deploy AI-driven risk stratification models in primary care to flag high-risk patients early.
  • Use wearable sensors and remote monitoring to track blood pressure, heart rate, and arrhythmias in real time.
  • Apply machine learning for drug discovery, accelerating identification of new molecular targets.
  • Integrate telehealth platforms and mobile apps for medication reminders, lifestyle coaching, and virtual cardiac rehab.

AI and digital tools can personalize prevention and treatment at scale, closing gaps between guideline recommendations and real-world care.

Next Steps

  • Advocate for policies that reduce unhealthy food marketing, expand smoking cessation programs, and improve urban design for physical activity.
  • Foster partnerships between researchers, tech developers, health systems, and communities to pilot AI-driven interventions.
  • Mobilize public awareness campaigns that emphasize simple heart-health metrics: know your numbers, move daily, eat smart, and never smoke.

Together, these strategies chart a clear path toward driving heart disease rates down and transforming cardiovascular health worldwide. Blueprints of the Heart

We are cartographers of the pulse,
drawing maps in veins and data streams,
AI shining lanterns on hidden currents—
so every beat becomes a beacon of hope.

In playgrounds and park lanes we gather,
feet falling in time with shared determination,
while algorithms learn our habits, our stories,
and whisper gentle reminders: “Check your numbers.”

Kitchens fill with low-sodium recipes,
gardens flourish under caring hands,
and digital voices guide the hesitant step
toward another breath, another dawn.

Together we code compassion into action,
melding human warmth with machine insight—
so every heart, once fragile and alone,
finds rhythm, resilience, and a home.

To learn better, I learn values of hearing, with heat of day in cool pool, enjoying joys of jungle, yelp call for help, who, where, sports groups competing with each other may have tunnel vision about heart disease and need help at the micro level of health care, we may wish to investigate each person needing help to prevent and reduce heart disease no matter how small from east to west, is everyone doing their best, explore each building, floor to floor, time for all to come to aid of all no matter how small, let all voices be heard, protect all from needs to prevent and reduce heart disease. 8/6/25

AI may help provide connected care with health reminders, techniques, evaluation of a personalized care plan, provide virtual health coaching to track goals with proactive continuous partnership to resolve immediate concerns, questions to prevent small issues escalation. With monthly plans, AI may help to adjust strategies to support progress or challenges. 8/7/25

Letting Love Lead: A Libra Season Evaluation on Health, Friendship & Faith

Date: September 22, 2025
Author: Desha Helen Baker
Theme: Challenges of Love | Libra Month | Compassionate Technology | Cherishing Behaviors

Today is Monday wrapped in affection, appreciation, and the gentle invitation of Libra season: to balance, to harmonize, and to let love lead the way. As I evaluate with my September 2025 DBSA Wellness Tracker, I feel energized and deeply thankful for the opportunity to grow in love, friendship, and whole health.

The Challenge of Love in Human Relationships

Libra, the sign of partnership and justice, reminds us that love feelings may develop with courageous acts of connection. In the purgatory of human relationships, where misunderstandings and emotional distance may linger, I am learning to encourage performance pacts: structured agreements to show up with kindness, curiosity, and fairness. These pacts help me grow where love is leading.

From the Welcome Team Guide by Dynamic Catholic, I’m reminded to seek “the will of God.” That means asking not just “What do I want?” but “Where is love leading me?” and “What matters to you—and why?” These questions open the door to compassionate care communication, where courage meets curiosity and emotional connection becomes possible.

Science of Well Being & Character Strengths

With the Science of Well-Being Rewirement Workbook, I am practicing new ways to develop character strengths:

  1. Kindness through cherishing behaviors and thank you rituals
  2. Love through help letters and form letters that express our deepest need to love and be loved
  3. Thanksgiving through daily evaluations and prayer
  4. Fairness through collaborative problem solving and fair goal setting

I thank God for love leading. I revisit the past 24 hours to notice when I was and was not the best version of myself. These evaluations help me learn, forgive, and change to achieve health results.

Health Technology & Community Empowerment

Love also leads through science. At the Indian River County Library, self-monitored blood pressure cuffs are now available for checkout, along with resource sheets that link to evidence based health practices. As Jimmy Clarity of the American Heart Association says, “Ensuring health equity is at the heart of our mission.” This initiative empowers individuals to take control of their heart health and bridge gaps in access.

I am integrating this with my Improve Communication Habit Recipe Reminder Card:

When I let love lead the way, I use effective communication techniques with science support and celebrate with positive thank you’s.

Friendship, Art & Public Spaces

My Happy Home Kindness Learning Adventure Project continues with:

  1. Riverside Park’s cardiovascular trail for heart health
  2. Vero Beach Art Museum’s “Art in the Park” Saturdays
  3. Libraries providing technology for creative learning and webinars
  4. X posts and email updates from federal, state, and county health agencies

These spaces are emotional ecosystems where love, learning, and belonging thrive.

Faith, Forgiveness & Creative Ways to Love

I feel concern for the limited scientific evidence for God, yet I remain open to exploring what God might be trying to say through events and people. I pray for renewal, reconciliation, and the courage to share God’s love creatively with confidence and comfort.

I lift up to God anyone I feel called to pray for today. I ask for wisdom and endurance for all of us, as we learn to grow where love is leading.

Evaluation Prompt:
What matters to you and why?
How is love leading you today?

Let us keep growing, together with affection and appreciation.

Health Technology

Getting to the Heart of Stroke program with the American Heart Association and HCA Florida Healthcare offers a toolkit with the Omron type of blood pressure monitor with the Libraries for Heart program.  The toolkit mission is to start my journey to leading a longer and healthier life. High blood pressure is serious yet manageable. Before Blood Pressure Measurement, the toolkit recommends to prepare with rest 5 minutes, comfortable, calm, relaxed, 2 readings 2 minutes apart before medications and before dinner in evening https://bit.ly/3UiAtSg, https://bit.ly/4b1xlCl, heart.org/hbp, heart.org/lifes8. With check is learning opportunities to change and control to minimize effects of high blood pressure with Healthy For Good American Heart Association list of factors with the toolkit encouraged 1 week of readings for accurate blood pressure measurement with LowerYourHBP.org, Omronhealthcare.com/app. In April 2025, I learned how to integrate the websites with apps with family Blood Pressure Monitor with Thriving Together: Empowering Healthier Communities: Social Determinants of Health and to share with others with returning the Blood Pressure kit to the North Indian River County Library with communication about needs for synchronization with Google Fit app and websites linkages of Indian River County Library website with the American Heart Association website with library Blood Pressure kits to follow information given in the Blood Pressure kit organized locally with Cleveland Clinic Florida.

With these heart healthy choices, I wish to evaluate how to encourage two sets of blood pressure readings paired with my smartphone before engaging in nutrition and exercise am and pm with 5 minutes rest and two blood pressure readings 2 minutes apart and input data into the Heart Rate Monitor - Pulse App - Apps on Google Play with AI Doctor support.

Sept 21 2025 9:45 AM 10:18 AM Normal 106/74 102/73 mmHg Sys/Dia 72/68 BPM Pulse Omron app

When I inputted this data into Heart Rate Monitor - Pulse App - Apps on Google Play with AI Health Assistant, I receive normal healthy blood pressure advice and measured 75 bpm pulse rate, 11:32 AM with positive scores of 27 % stress level, 81 ms Heart Rate Variability, 37 % Energy I learn the importance of balanced diet with fruits, vegetables, whole grains, 150 minutes moderate aerobic exercise/week with muscle strengthening exercises, stress reduction if needed with meditation, deep breathing exercises, yoga, new hobby, mindfulness. I learn the importance of blood sugar tests with ADA Diabetes Risk Test. I learn the importance of BMI with Hypertension Risk Test, Hyperlipidemia Risk Test and Stroke Risk Test. I learn the importance of specific neck exercises to reduce tension and build muscle strength with the Cervical Spondylosis Risk Test.

AI Health Assistant's Answers to My Questions

1.  Do you recommend check out a blood pressure kit from Brackett Library in Indian River County, Florida to encourage joyful, culturally respectful experiences with heart healthy choices and deepen neighborhood connections? AI Health Assistant's answer:

While I can't speak specifically to library policies or community initiatives, regularly checking your blood pressure is an excellent practice for maintaining heart health. If your library offers a blood pressure kit, using it can foster awareness of your cardiovascular health and encourage discussions about heart-healthy choices in your community. If you have more specific health questions or need advice related to blood pressure management, feel free to ask! Heart Rate Monitor - Pulse App - Apps on Google Play

2. Do you recommend friendship with Blood Pressure kits? AI Health Assistant's answer:

It seems there might be a misunderstanding in your question. Blood pressure kits are medical devices used to monitor blood pressure, not entities for friendship. It's important to regularly check your blood pressure if you're managing hypertension or other cardiovascular issues. If you have questions about how to use a blood pressure kit or how to maintain a healthy blood pressure, I'm here to help! For non-health-related inquiries, I recommend seeking other sources. Heart Rate Monitor - Pulse App - Apps on Google Play

  3. What do you recommend to do if someone is interested in my blood pressure kit from the Brackett Library of Indian River County, Florida? AI Health Assistant's answer:

If someone is interested in your blood pressure kit, it's a good idea to share how you use it and the importance of monitoring blood pressure for health. You can encourage them to check it out at the Brackett Library if available or discuss any questions they may have about maintaining a healthy blood pressure. Key messages to share

  1. Friendship and empathy make learning stick: organize heart health activities as shared experiments to encourage creative learning for health results.
  2. Small tech steps matter: teach correct cuff technique, how to log readings in an app, and when to seek care.
  3. Celebrate cultural diversity with language, foods, and social examples to evaluate community.
  4. Let AI assist: Microsoft AI may help draft outreach, organize reminders, and analyze aggregated trends to improve engagement.

Libraries are trusted community hubs where people gather to learn, connect, and access validated health resources. A local blood pressure kit checked out from Brackett Library gives participants practice with monitoring, helps bridge access gaps when clinics are closed, and supports follow up tracking for better health conversations to achieve health results. Use of established wellness tools and trackers strengthens program credibility and helps participants turn readings into actionable goals.

Let love lead our learning this Libra month. At Brackett Library I checked out a validated blood pressure kit and explored how monitoring, friendly encouraging, and smart reminders may help neighbors make heart healthy choices. With Microsoft AI support, library media, and culturally respectful programming, we may turn data into care, and care into stronger friendships and healthier communities.

From Awareness to Action

Respectful Greetings and welcome. Thank you for reading this blog post. I wish to encourage public education, partnerships and eLearning with Mental Health America. I wish to help and facilitate Nurturing Belonging, Emotional Well-being, and Safety in a New School Year. I encourage participation and feel appreciation for kind sharing. I wish to help people to feel safe and respect culturally diverse backgrounds of our communities. I learn the importance of community love with relationships to support making life work for everyone. Trauma may show up differently with each individual and we may learn how to talk through any signs of negativity with a curious mindset. I feel encouraged to hold space of compassion with kind demeanor with this blog post. If more than one person has a similar concern, peer support groups with friends checking in on friends with resources may help.
Mental Health America Conference | Mental Health America may help to advocate, heal and learn with our communities. I feel thankful with respect for learning together with the new school year to improve health results. I learned how different pathways are based on people. Healthy Choices to Prepare for “Heart Health”

Elevate National Learning Forum | Tuesday, October 14, 2025, 1:00 PM – 2:00 PM ET

Evaluate Session Details and Objectives

  1. Topic: Heart Health
  2. Aim: Explore strategies, data tools, and best practices to enhance cardiovascular care in community health centers.

Gather Your Center’s Heart Health Data

  1. Pull recent metrics on hypertension prevalence, lipid screening rates, and BMI tracking.
  2. Identify two to three priority gaps or challenges your team faces in cardiovascular care.
  3. Prepare a one-slide summary or talking points to share during discussions https://copilot.microsoft.com/shares/24rRZvWUzgF3cJ85Q1haK

Evaluate Heart Health Resources

  1. New high blood pressure guideline emphasizes prevention, early treatment to reduce CVD risk | American Heart Association American Heart Association (@American_Heart) / X
  2. About the Division for Heart Disease and Stroke Prevention | National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) | CDC
  3. Heart Health Hub - NACHC

Plan for Active Participation

  1. Organize for the full hour and minimize distractions.
  2. Have a notepad or digital document ready for key takeaways and action items.
  3. Prepare questions or case examples to pose during live polls and breakout discussions.

Arriving prepared will help you maximize the session’s insights and leave with a concrete action plan for advancing heart health at your center.

Refresh Your PRAPARE Foundations

  1. Revisit the original PRAPARE (Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences) assessment tool to have context for the 2.0 updates with Elevate eJournal by nachc.com - Issuu
  2. Note any challenges your team has faced in implementing social determinants of health (SDOH) screening. https://copilot.microsoft.com/shares/U8T3BNsAhueYUNfUPgWLt These pain points may inform your questions during the forum. My closest location is TCCH Sebastian. When I visited TCCH Sebastian 9/10/25, I learned the importance of understanding how the PRAPARE assessment tool is only one of many assessment tools.

Map Out Your Nonclinical Data Landscape

  1. Create a one-page inventory of the non-clinical factors you currently capture or miss:
    1. housing stability
    2. education and literacy
    3. access to healthy food
    4. transportation barriers
    5. social and community context
  2. Identify your data collection methods (paper forms, EHR fields, patient portals) and highlight gaps where you want guidance on best practices. I learn with this blog post, X posts with email to encourage validated sources of health information. The technology used to collect data in community health centers with clinical data with health providers may create excess stress and therefore lose reliability for health results. I have learned about concerns for how excess stress may develop with different types of technology and the importance of updating my technology to reduce and balance excess stress with our ecological system of learning with God conundrums of where love is leading and how to encourage health relationships with all. With excess stress, identifying the most important factors to reduce stress depends on understanding how different factors affect each other and create patterns of behavior over time for each individual.
  3. Assemble any reports or dashboards you’ve generated on SDOH metrics, having real numbers accessible may spark more targeted discussion. Thriving Together: Empowering Healthier Communities: Heartwise Sebastian

Plan Your Post-Session Next Steps

  1. Organize 30 minutes the following business day to evaluate your notes and highlight three key actions.
  2. Circulate a one page summary to your team, including links to session slides or recordings https://copilot.microsoft.com/shares/1A2qVf4qa22P4SGSi53w5
  3. Incorporate forum insights into your workflow: update your EHR fields, update your screening protocol, or pilot a new community referral process https://copilot.microsoft.com/shares/9ChLQd6WPfUS8zkuoTXp8. With Week 1 of Heart Wise Sebastian community health campaign, I learned challenges of love.
Challenges of Love 9/16/25
Learning healthy choices
I learn the importance of respect for our cultural diverse backgrounds
With empathy and joy for healing to health
with stable homes, healthy food with harmonious movement
I feel love, affection and appreciation

To move from awareness to action

With registering early, refreshing your SDOH background, organizing your data, and engaging with purpose, you may encourage healthy transformative practice. Mark your calendar for October 14 at 1 PM ET and get ready to elevate your approach to PRAPARE 2.0 and nonclinical factors of health or social drivers of health depending on the context of your conversation. *Please note PRAPARE 2.0 is only one tool among many assessment tools if not available at your local community health center.

5. “We missed you at the session, do you wish to make up or help finding a better time? You may reply MAKEUP or TIME.”

6. "Thank you for your encouragement. When do you prefer to receive support of healthy choices, weekly or daily?"

7. "Good effort, [Name], you met your goal this week. Want to book an AI nurse check in to evaluate results? Reply BOOK."

10/12/25 With the focus on AI, I wish to evaluate how to learn with AI with the community events with the National Centers for Community Health with Mental Health America with this blog post. Google apps synchronize with Google Calendar yet follow up with informational learning is supported with Microsoft AI. With numerous conference events, I may input my learnings for each community event I attend and let AI apps evaluate my learnings. Radiant Intersection of Summer Solstice and Learning 2025Temporary Art

10/14/25 I have learned the importance of heart health with the NACHC's Elevated Learning Forum "Heart Health" webinar and feel concern for how to prevent and reduce heart disease because heart disease is the leading cause of death in Indian River County, Florida. How do you recommend triage for Immediate Health Concerns to integrate the process improvement strategies highlighted in the recent NACHC Heart Health webinar?

What do you recommend for Immediate Health Concern Protocol?

  1. How to Identify triggers? How to support patient intake prompts to quickly surface the most urgent symptom or concern?
  2. How to prioritize the highest concern? How may triage flow emphasize identifying the single most critical issue first, before layering in secondary needs?
  3. How to support navigator engagement? Whenever possible, with immediate health concerns, how to encourage a call to a navigator within 30 minutes? If the patient cannot connect, how to connect to AI to evaluate urgency and, if necessary, escalate to possible emergency services?
  4. How to develop documentation & outreach? I have received your alert about the importance of hypertension control in our communities. I am calling now to check in, who may help?

Integration with Microsoft AI Support

  1. AI may assist in evaluating the concern, suggesting next steps, and flagging potential emergencies.
  2. Outcomes may be logged in a structured format for both clinical and quality improvement review.

Process Improvement with Heart Health Webinar Alignment
Drawing from the NACHC Heart Health session, how to integrate

  1. Accurate measurement practices (repeat readings, calm environment) into navigator scripts.
  2. Data-driven improvement loops by linking triage outcomes to hypertension and cardiovascular risk dashboards.
  3. Care escalation algorithms that align with the Three Pillars approach to hypertension control.
  4. Teach-back loops for patients to confirm understanding of next steps, reinforcing adherence and safety.

Next Steps for Collaboration

  1. Co-develop a scorecard that tracks both immediate concern resolution and alignment with heart health measures (BP control, statin use, tobacco cessation).
  2. Pilot a navigator + AI co-triage model with real-time feedback loops.
  3. Share learnings with the Elevate/NACHC community to strengthen collective practice.

Here is a one-page triage + improvement scorecard template aligned with the Immediate Health Concern workflow and the NACHC Heart Health process improvement lens.
